Irinotecan is a semi-synthetic derivatives of camptothecin which can be specifically combined with the topoisomerase I. Topoisomerase I induced reversible single-strand breaks, so that double-stranded structure of DNA unwinded. Irinotecan and its active metabolite SN-38 with topoisomerase I-DNA complex binding, thereby preventing the fractured single-stranded nucleic acid to be connected. Existing research suggests that irinotecan cytotoxicity due to DNA synthesis process. Replicase and a topoisomerase I-DNA irinotecan (or SN-38) triple complex interactions, causing DNA double-strand breaks. Mammalian cells can not efficiently repair the DNA double-strand breaks.
